How to Request Your Medical Records

• Patients: Request online with a valid photo ID (Driver’s License, Military ID, or State ID). Legal representatives may need additional documentation. Processing takes 24–48 hours.
• Physicians (Urgent Requests): Fax requests on office letterhead with authorization to 940-495-4137 for same-day processing.
• Third Parties (Insurance, Attorneys, Disability Services): Mail requests to:
